CR 08-240 - New attributes for publisher of a book name and publisher of a book GLN

This CR creates 2 new attributes which will also be deployed as Fast Track to communicate the name and GLN of the publisher of a book or publication. The Publisher of Book Fast track attributes are being entered on advice from the Books Workgroup to allow for a solution until the Book extension is available. As an AVP it is a Simple LOI 1 and in the long term the Book WG is already dealing with this as an attribute
